We are currently accepting referrals for plan and self-managing NDIA participants of all ages. We regret that we are unable to accept NDIA managed plans at this time.
We offer a predominantly mobile service and can arrange visits at a location of your preference (travel fees apply), alternatively, we have a small clinic space located in Rivervale, WA, for you to attend.
Providing comprehensive assessment and intervention for: sleep; dressing; personal hygiene; tasks such as teeth brushing and grooming; bathing.
Completion of comprehensive functional assessments to determine the need for additional services and support.
Providing assessment and intervention for: cooking skills; road safety; money handling and budgeting; emergency and safety skills; cleaning and laundry skills; organization, time management and executive functioning skills; driver readiness (please note, this does not include formal driver assessment)
Assessment of specialized equipment such as wheelchairs, shower commodes and bathing supports, hoists, alternative seating, night-time positioning, electrically adjustable beds and pressure mattresses. Including the completion of mid cost and high-cost Assistive technology applications for NDIA.
